Family Health Care was founded to serve the Shoreline / north-Seattle region as a full-spectrum primary-care home for families. Led by Dr. Dinh Tran, MD, we opened our doors at 1507 NE 150th St, Suite A, Shoreline, WA, with the goal of delivering healthcare that is personal, accessible, and built on long-term relationships.
From day one our vision has been to deliver “complete family health” — by bringing together preventive wellness, chronic-disease management, mental health, and advanced services like medical weight-loss and concierge medicine under one roof.
Our mission is to empower our patients to live their healthiest, fullest lives — not simply treating illness when it appears, but partnering with each person across every stage of life. We strive to build a healthcare home where prevention is as central as treatment, where each patient is known as a person (not a chart), and where our team helps patients navigate their health proactively, confidently, and holistically.
At Family Health Care (Shoreline), our philosophy centers on three tenets:
Continuity of care: We see you, your spouse, your children, your aging parents — offering a medical home that spans lifetimes rather than siloed episodes. Integration of body & mind: We believe physical health and mental health are deeply connected. That is why we offer medical care (diabetes, hypertension, heart disease, etc.) alongside mental-health services under one roof. Personalization access: Through offerings like concierge medicine and integrative weight-loss management, we tailor care to your needs and lifestyle —not a one-size-fits-all model.
One distinctive service is our concierge medicine model and annual-membership access. The website states: “Our annual memberships provide a new way to manage your health. Our subscription-based model offers unlimited access to personalized primary care and weight-loss services.”
In addition, the practice offers integrative medical weight-loss management (alongside chronic-disease care) which many standard primary-care clinics do not. This sets us apart by blending preventive wellness, lifestyle change, and medical oversight — all under one roof.
We approach care differently by:
Taking the time to listen and understand each patient’s story, lifestyle, challenges and goals (rather than simply reacting to lab values or symptoms). Building a long-term partnership with patients — your health is our shared journey, not a series of disconnected visits.
Offering more than the traditional clinic: we bring weight-loss, geriatric, mental- health and chronic-disease management services together so patients don’t have to bounce between multiple specialists. Giving enhanced access — via concierge membership, possibly faster appointments or more time with your provider — so you feel truly supported.
Every day brings something meaningful. A typical morning might start with reviewing the schedule and preparing for a mix of appointments: a child with asthma, an adult with new-onset diabetes, a senior managing congestive heart failure, and a weight-loss follow-up. Mid-day might include a same-day sick visit, an IV hydration therapy, or mental-health check-in. Afternoons could bring proactive wellness visits (screenings, annual check-ups) or concierge-member consults. Our team meets regularly to review patient panels, follow up on care plans, coordinate between mental-health and medical services, and ensure no one falls through the cracks. While there is structure, the variety means it truly is an “adventure” — each patient brings a unique need, story and opportunity to make an impact.

Dr. Tran’s father battled prostate cancer, which made him witness firsthand how fragmented care can leave patients frustrated. The disconnect between patient’s goals of care versus treatment plan, all while navigating communication gaps in a complex web of healthcare logistics, led to feelings of defeat and exasperation. That experience shaped his commitment to integrative, continuous, whole-person primary care.
Each year our team participates in a Community Health Fair where we provide free wellness screenings and education — one year we met a young mom who’d been delaying care out of fear. After connecting with us she began a chronic-disease plan and today is thriving. That moment reinforced why we don’t just see patients — we serve neighbors, and it motivates us to break down access barriers.
From a young age Dr. Tran was drawn to medicine because of his family’s immigrant background and the gaps he witnessed in access and continuity of care. The spark to make a difference in people’s lives has never faded — today he still wakes up each morning energized by the opportunity to build enduring relationships, help someone avoid a serious complication, or enable someone to reclaim their health and wellness through our integrative model.
